Thank you to those who were able to participate in our 2023 Resident & Family Engagement Survey. Our team has reviewed the feedback and would like to ask some more questions to assist with guiding our quality improvement annual plan in the home. We appreciate you taking the time to provide your feedback. It is very important to us.

Please answer the questions provided with your loved one, or if they are unable to answer the questions themselves; please answer on their behalf based on your observations during your time with your loved one. All feedback is important to us and we use the feedback to make changes in the coming years to continue to improve the quality of care for all residents in the home.  

Please respond to the questions provided in the survey either as the resident or on behalf of the resident, if you are a person of importance. Responses for the resident should reflect your observations and experiences during your time with them. Your feedback is highly valued and will help us make meaningful improvements to the quality of care in the years ahead.

Persons of importance may include anyone significant to the resident, such as family members, Power of Attorney (POA), Substitute Decision Makers (SDM), essential caregivers, and close friends.

We are updating our Strategic Plan for 2026–2029 and envision a unified approach that brings together the Pines and Fairvern Strategic Plans into one comprehensive Long-Term Care Strategic Plan for the District of Muskoka. Your insights are essential as we move through this work. Thank you for taking the time to help shape the future of long-term care in our community.
